CARE COORDINATOR: 

  • Provide intensive home-based services in partnership with family to create a Family Plan, make referrals, facilitate access to services and monitor to ensure that all components of Family Plan may be accomplished .  
  •  Receives cases assigned by Program Director and maintain a case load of 15-20 families.
  • Collects pre and post data on families and enter into data base.
  • Works effectively as team member and provides assistant to the mental health consultant.
  • Convenes child/family specific team, provides facilitation of Family Plan of services.
  • Enter Family Plan in data base and track service access, barriers and gaps.
  • Work with families in scheduling service appointment and monitor service utilization.
  • Attends staff meetings, conferences, and workshops regularly to upgrade professional skills.
  • Maintain filing system for all cases and keep records of all contacts with families and providers.
  • Work with the family to identify long term, on-going community helper from among their service providers and natural helpers.
  • Through on going relationship building with the family, help facilitate nurturing, contingent, parent child interactions.
  • Participate in group supervision/case conferencing and scheduled individual supervision weekly.

BA in social work, psychology, human services or related field required, minimum of 2 years working with children, youth and families in community and home based settings, knowledge of Miami Dade community & resources preferred, excellent written and oral communications, strong computer skills, bilingual - English / Spanish  and or English/Creole  preferred but not required.   (6) Position 's  F/T. Monday - Friday  (flex-time) .  Salary $32k and an excellent benefit package.  

 

MENTAL HEALTH CONSULTANT:

  • Receives cases assigned by Program Director and schedules home visits with families for intake and assessments.
  • Collects pre and post data on families.
  • Carry clinical case load, providing in home parenting and mental health consultation, as part of a team with a care coordinator.
  • Works effectively as team member and provides consultation to service providers and court staff.
  • Make recommendations based on assessment of family strengths and needs.
  • Attends staff meetings, conferences, and workshops regularly to upgrade professional skills.
  • Maintain filing system for all cases.
  • Conducts weekly home visits/face to face contacts with family members.
  • Provide support to families in the area needs i.e., parenting, conflict resolution skills and child behavior management skills.
  • Participate in group supervision/case conferencing and scheduled individual supervision weekly.    

 

MA in social work, psychology, human services or related field required, minimum of 2 years working with children, youth and families in community and home
based settings, knowledge of Miami Dade community & resources preferred,  experience working with children of high conflict families and domestic
violence,  excellent written and oral communications, strong computer skills, bilingual - English / Spanish  and or English/Creole  preferred but not
required.   (3) Position 's  F/T. Monday - Friday  (flex-time) .  Salary $42k and an excellent benefit package.
